          __ Eagle Scout Leadership Service Project Workbook #18-927A or computer generated equivalent used                      (If utilizing  regular workbook, all available space in the book is used before adding attachments.)

          __ Signature of Religious institution, school, or community representative who will benefit from the project.

          __ Signature of Scoutmaster/Coach/Advisor.

           __ Signature of Unit Committee member.

Project Description:

Planning Details:

              __ Complete description of the present conditions. (pictures, maps, drawings, etc.)

             __ Complete description of the methods used to complete the project.

             __ Project work properly organized.

             __ Sufficient demonstration of leadership.

             __ Materials required for the project:

             __ Complete list of necessary materials and cost.

             __ Sources of materials to be secured. (lumberyard, nursery, etc.) 

             __ Sources of funding identified and prospective presentations prepared. (fundraisers, donations, letters to                         organizations/individuals, etc.)

      __ Resources required for the project:

             __ Complete list of all resources needed for the project. (tools, transportation of materials and personnel, etc.)

             __ Sources identified for securing the resources. (individuals, rentals, etc.)

             __ Project helpers necessary to complete the project:

             __ List of special skills needed and prospective workers.

             __ List of sources necessary to secure these workers. (scout troop, parents, church youth group, professional                      resources, etc.)

             __ List of sequence of events, leading to the successful completion of the project.
